Open your invitation
Follow the survey link in the provider’s message. Check the survey title and introduction so you know what feedback is being requested.

Answer the questions
Complete questions marked with an asterisk. Depending on the questionnaire, answers can use text, single or multiple choices, numbers, dates, yes/no controls, or rating scales.

Review your response
Check your answers before submitting. A checkmark beside a field indicates that an answer is present; it does not mean the whole survey has been sent.

Submit the questionnaire
Select Submit Survey and wait for the result. Correct any highlighted required-field errors. A successful submission displays a thank-you message.
Leaving and returning
Complete and submit the survey before closing it when possible. Do not rely on automatic progress saving or expect a draft to appear on another device. If the page offers Remember progress on this device and a saved-progress indicator, that is local browser-session progress, not a submitted response. Clear saved progress clears the locally retained answers. You still need to select Submit Survey to send the questionnaire.If the link or submission fails
A questionnaire can be associated with your invitation, company, or support ticket. Depending on the provider’s configuration, submitted feedback can be used in reports, trigger follow-up, or appear in the linked ticket history.
